Allow different types of custom fields for CQ RP

It is common to need extra fields for CQ RP which are not decimal. For example customers usually use Skelta to submit the CQ surveys and we need to save who has initiated the survey and who has approved it and some other information. Currently none of these is possible and the only field we can add extra info is comment whcih is not enough and convenient.
