Ignore folders when showing cause/classification/effect


[Problem Summary]
According to the last bug comments in CE-1627, the issue was fixed such that "Where as, the filter form will sort Cause, Effect and Classification in the alphabetical order but withing the lookuplist/folder".

This was not an issue for the customer but with more and more Cause code added into the project and in different folders as the project evolves, this creates a problem for users when they try to search for specific Cause. The users do not know exactly how those cause codes are configured in the project(which one is in which folder), it becomes very difficult for users to locate the Cause code of interest to them as they are seemingly sorted randomly.

Very hard to locate the cause code of interest as the way the sorting works is counter intuitive.
